Used when the artist has made a severe error in proportion, drawn a limb in an impossible position, or some other unacceptable irregularity in a character's form.

Merely having a anime head is not enough. An ear being on the neck, an arm bent at an angle beyond breaking point, or two left or right feet, are all good examples.
