Sankaku Channel/"Meta" tags introduced

A new tag type has been introduced in the form of "meta" - this is primarily intended to cover request and moderation related tags. As well as being a logical addition it will also help keep these tags out of lists they are not really supposed to appear in.

The scope of this tag and any potential additions to it, their position in tag lists (meta are presently at the bottom) and even their colour or indeed the addition of other tag types, are all open for discussion, so if anyone has any comments they can feel free to make them here.

Following on from these, the "medium" tag has been added - covering media and formats of all kinds.
