It is possible for a picture to depict someone urinating with there being no visible urine in the picture, for example when sat on a toilet or stood against a wall.

The converse is even more obvious - plenty of urine can feature in pictures with nobody actually passing water.

I would never argue for the other direction.

I was aware of the example you gave beforehand, it is quite a corner case because if there is no additional contextual information (like speech or audio indicators) you cannot even infer that any urination is happening.
There is another counterexample, namely the depiction of the act without the associated fluid, something like the Manneken Pis.

In the vast majority of cases the implication holds though, and often one of the tags is forgotten when both should be applied. Ideally the system should simply suggest the other when one is added but i assume that is not possible right now...

I am not referring to the search suggestions but rather to suggestions when people tag the images. As you can see in the results of the search you linked most of those pictures actually show urine but lack the tag.
