Sankaku Channel/Encoding issues

I am starting this thread to report encoding issues, mainly ampersand entities and exposed UTF-8 encoding in tags. (Search: *&*;*) These tags are sticky so aliasing is used to correct this. If it is possible to edit/remove such a tag by normal editing please do so.

In addition to getting such aliases approved we'll also be looking at how they are introduced in an effort to cut down their creation (this seems to have been largely accomplished) so by all means report any such issues.
